The supports under this service
We tailor the mix to you, but here’s what this service typically covers. Each maps to an NDIS support category your plan may fund.
Community nursing
Registered & enrolled nurses providing care at home — medication support, wound care, continence and ongoing health monitoring.
Exercise physiology
Tailored movement and exercise programs to build strength, mobility and confidence, designed around your goals and abilities.
Complex & high-intensity supports
Specialised care for higher health needs, delivered safely by clinicians trained in high-intensity daily personal activities.
Assessment & care planning
Qualified clinicians assess your needs and build a clear, person-centred care plan — reviewed regularly as things change.
Where this sits in your NDIS plan
Allied health and therapy supports like exercise physiology are generally funded under Capacity Building — Improved Daily Living. Community nursing and clinical care are usually funded under Core Supports, including high-intensity daily personal activities for participants with more complex health needs.
Not sure where this fits for you? That's completely normal — we'll help you read your plan and work out what's possible, with no obligation. Our care works with how your plan is managed, whether you're self-managed, plan-managed or NDIA-managed.
